approved provider.) Who are we? At Autism Care Partners (ACP), our mission is to empower families to reach their potential through early diagnosis and exceptional, innovative, interdisciplinary care. Our collaborative teams provide comprehensive care throughout all stages of a child's development by offering these services: Applied Behavior Analysis, Speech Therapy, Occupational Therapy, Social Work, and Diagnostic Evaluations (service availability varies by location). What you'll be doing atAutism Care Partners:
Regular and reliable attendance is an essential function of this position. Provides speech and language services for patients as ordered by the patient's physician, or authorized at IFSP (EI ONLY) Evaluates speech and language disorders and/ or swallowing, provides differential diagnosis of speech, language and voice and swallowing disorders utilizing formal and informal assessments Utilizes therapeutic devices and other means to maintain or restore maximum speech and language capabilities Plans, instructs and supervises the patient, family and other health care providers in the use of specific verbal/ physical exercises and equipment (as needed) to facilitate speech and language skills Assists with referral of patients to speech and audio logical clinics, dentists or other related services Provides counseling and guidance to speech/ language and hearing impaired individuals and their families Submits an initial assessment which indicates level of patient functioning/ disability, short and long term goals. Submits progress notes after each home visit which describes treatment given, modalities used, progress observed and patient response to treatment Submits a summary of a patient progress towards goals of therapy (monthly progress notes for EI only) Submits a discharge summary at time of discharge of patient from therapy Maintains communication with physicians and other involved in the patient's care keeping in contact through written reports, attendance at meetings and documented telephone calls Coordinates care through the nurse Refer to speech therapy policies for required communication and documentation schedule Participates in interdisciplinary staff case management conference and in-service program as required Education and Experience Master's degree in Speech-Language Pathology Certificate of Clinical Competence (CCC) by the American Speech-Language Hearing Association (ASHA) or in pursuit ofASHA CCC
State license in Speech-Language Pathology (dependent on locations conducting therapy) Proficiency in the field of autism/developmental disabilities Basic computer skills required - includes understanding of Microsoft Office, Windows, Central Reach, and other software as needed Familiarity with AAC is preferredOUR MISSION
