DEPARTMENT
Rehabilitation/Speech Pathology
SCHEDULE
Day shift, 8-hr. shifts, 8:30 am - 4:30 pm (per diem as assigned) occasional weekends & holidays
RESPONSIBILITIES
Evaluation & Planning. Perform a complete initial evaluation of the referred patient; develop and implement treatment plans, including appropriate short- and long-term goals; involve the patient, family, and staff in the implementation of the treatment program; re-evaluate patient functional status progressively; advance patient toward maximal performance and functional independence Documentation. Maintain a complete documentation of patient evaluations, treatment, progress, and discharge status; send required information to appropriate professionals Collaboration. Actively participate in rehabilitation team efforts to meet patient needs, including counseling and teaching patient and family, discussing changes in treatment programs with MDs, coordinating healthcare plans with other involved professionals, and assisting in discharge planning as appropriate
REQUIRED QUALIFICATIONS
Master's degree in Speech-Language Pathology or Communication Disorders (may be MA, MS, Med)
Required Licensure/Certification Skills:
New York State Licensure Minimum of nine (9) months of supervised work experience (completion of CFY), or participation in supervised clinical fellowship, for a minimum of 36 weeks under a NYS licensed Speech Pathologist, followed by the NYS licensure examination and achievement of NYS license Compassionate, warm, and patient-focused Excellent organization and planning skills
PREFERRED QUALIFICATIONS
One (1) year of experience with the frail elderly population is preferred

REQUIREMENTS
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting. For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. Any physical requirements reported by a prospective employee and/or employee's physician or delegate will be considered for accommodations.

14626 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
