Job Description Summary:
- To competently interpret the patient's history, perform a complete diagnostic evaluation, communicate findings, and develop treatment plan; to initiate therapy and progress the patient's through the treatment plan; to educate and counsel patient, family members, and staff, depending on their needs.
Responsibilities And Duties:
- Direct Patient Care 90-100% Evaluates and treats patients: After provider referral, evaluates patient communication, cognitive, functional status and/or swallowing, then develops, and implements plan of care based on evaluation findings and appropriate treatment techniques and initiates patient treatment.
Department Functions:
Supervises patient assignments and duties of the rehab techs, as applicable. Is involved in department quality improvement through data collection and participation in team and staff meetings. Assumes duties delegated by management in the absence of department management. There is an expectation that an employee in this position may be requested to work at care sites other than their home care site.Education/Mentoring:
Takes active role in orienting new staff members. Serves as clinical instructor for affiliating SLP students and/or CFY placements. Serves as an advocate for the discipline on the multidisciplinary team. Represents department at problem-solving and committee meetings. Master's Degree:
Speech Pathology (Required)ASHA CCC-SLP
- ASHA Certificate of Clinical Competence for Speech-Language Pathologists (CCC-SLP)
- American Speech Language Hearing Association, DL-HC
- Driver's License only if in a Home Care department
- Department of Motor Vehicles, SLP
- Speech Language Pathologist
- American Speech Language Hearing Association
Additional Job Description:
- American Speech Lang Hearing Assoc and Cert of clinical Competence Speech Lang Pathology is required.
