Job Description

Elumina Health is a locally owned and operated agency in Columbus, Ohio and we are adding to our team due to growth! We are searching for an energetic Speech Language Pathologist to join our team in Scioto County. The ideal candidate will be patient care focused, organized, flexible, comfortable using a computer, and possess excellent communication & customer service skills. The Speech Therapist is a qualified professional person who directs, supervises, evaluates and provides speech therapy services to clients in the home as prescribed by the physician.
Qualifications:
  • Graduate of an approved speech therapy curriculum.
  • Possesses a current license to practice as a speech therapist in Ohio.
Responsibilities:
  • Evaluates client's swallowing, speech and language abilities, both defects and assets and performs re-evaluations.
  • Plans and provides rehabilitative services for speech and language disorders.
  • Records type of treatment and client's reaction to it on clinical/ progress notes which are submitted weekly to the Agency.
  • Maintains appropriate records on all clients, including summary and discharge reports.
  • Selects and administers therapeutic techniques and materials.
  • Instructs and counsels other health team personnel, the client and family members in methods of assisting the client in improving, correcting and/ or accepting his or her disabilities.
  • Attends care conferences and other Agency meetings as necessary.
  • Participates in the development of and periodic review of the physician's Plan of Care.
  • Communicates effectively with all those providing care.
  • Participates with client, staff, and physician in discharge activities and completes the speech therapy discharge summary.
Organizational relationship:
  • Reports directly to the Director of Rehab & Therapy.
Functional Abilities:
  • Must be able to read 12 point type or larger.
  • Must be able to communicate verbally and in writing in a manner understood by most persons.
  • Must be able to travel to client's residences.
Special requirements:
  • Must have a reliable car, car insurance and a valid driver's license.
