3x8 Hour Days (Wednesday-Friday, 8:30 AM - 5:00 PM)

4x10s or 3x12s schedules are not available Role Overview We are seeking a dedicated Home Health Speech-Language Pathologist (SLP) to provide one-on-one patient care in a home health setting. This role focuses on delivering high-quality speech therapy services, managing patient caseloads efficiently, and ensuring accurate documentation using standardized systems. Key Responsibilities Provide speech-language therapy services to patients in home health settings Manage a caseload of up to 6 patients per day (productivity standard: 30 points) Perform patient assessments, develop care plans, and track progress Complete accurate and timely documentation using required systems Travel to patient homes and healthcare facilities within the assigned region Collaborate with interdisciplinary care teams to ensure optimal patient outcomes Maintain compliance with healthcare regulations and organizational standards Required Qualifications Minimum 1 year of skilled home health experience Active SLP licensure (or pending license accepted) BLS (Basic Life Support) certification required Experience with OASIS documentation ( required ) Valid driver's license and reliable transportation
