Friday Hours:
24 Hours Per Week (3x8-Hour Shifts)
Responsibilities:
Provide speech, language, swallowing, and cognitive therapy to adult and geriatric patients in their homes. Manage a home health caseload and complete scheduled patient visits. Perform comprehensive clinical assessments and develop individualized care plans. Complete OASIS documentation accurately and efficiently. Document patient progress, outcomes, and care coordination activities in EMR systems. Collaborate with interdisciplinary healthcare teams to support patient care goals. Educate patients and caregivers regarding treatment plans and therapy goals. Manage daily travel and scheduling between patient visits. Maintain compliance with home health regulations and agency standards.
Preferred:
Prior experience with Epic EMR software.
Requirements:
Minimum 1 year of recent skilled Home Health experience. Active Oregon SLP License (pending Oregon license accepted). Current BCLS (AHA) certification. Direct OASIS documentation experience required. Valid driver's license and clean driving record. Reliable personal transportation required. Strong communication, documentation, and independent clinical decision-making skills.
