Physical Therapist (PT), Occupational Therapist (OT), Speech-Language Pathologist (SLP), Physical Therapist Assistant (PTA), Occupational Therapy Assistant (COTA), or Speech-Language Pathology Assistant (SLPA)Job Summary We are seeking compassionate, motivated, and patient-focused rehabilitation professionals to join our growing team. We are currently hiring PTs, PTAs, OTs, COTAs, SLPs, and SLPAs to provide high-quality therapy services in both our outpatient clinic and patients' homes. This position offers the opportunity to work in a collaborative, multidisciplinary environment while providing individualized care that helps patients improve mobility, communication, swallowing, functional independence, and overall quality of life. Responsibilities Evaluate patients (licensed therapists) and develop individualized treatment plans. Provide skilled therapy services in both outpatient clinic and home settings. Implement evidence-based interventions appropriate to your discipline. Assist patients in achieving their functional, communication, mobility, cognitive, and swallowing goals. Educate patients and caregivers on treatment techniques, home programs, and strategies to maximize progress. Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and in a timely manner. Monitor patient progress and communicate changes with supervising therapists, physicians, and other healthcare providers. Collaborate with an interdisciplinary rehabilitation team to deliver comprehensive patient care. Maintain compliance with HIPAA, state licensure requirements, payer guidelines, and company policies. Travel to scheduled home visits within the designated service area. Qualifications Current state license or certification as a PT, PTA, OT, COTA, SLP, or SLPA. Graduate of an accredited therapy program. Current CPR certification (preferred). Valid driver's license, reliable transportation, and auto insurance for home visits. Excellent communication, organizational, and clinical skills. Ability to work independently while functioning as part of a collaborative team. Home health experience is preferred but not required. $60,000.00 - $85,000.00 per year
