Mesa Springs Healthcare Center is a modern, state-of-the-art skilled nursing facility and Independent Living community, located in the Far Southside neighborhood of Abilene. We are along Buffalo Gap Rd, between Antilley Rd and Beltway S. We feature 89 beds and beautiful spacious rooms in either private or semi-private settings. The Speech Language Pathologist is responsible for providing rehabilitative therapy to patients upon referral for communication, cognitive and/or swallowing disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services. This position reports to the Director of Rehabilitation Services.Duties:
Screen and evaluate patients with communication, cognitive, or swallowing disorders, developing appropriate care plans following regulatory and clinical standards. Deliver rehabilitative treatment to patients with communication, cognitive, and/or swallowing disorders, ensuring compliance with regulatory and clinical practice requirements. Evaluate treatment outcomes, modifying services based on patient evaluation and making referrals as needed. Provide consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders. Generate comprehensive discharge summaries in adherence to regulatory and clinical requirements.Qualifications:
Must be ASHA certified (CCCs) Must be currently registered/licensed with applicable state One year's experience in long-term care or related setting preferred Three years' experience holding active CCCs prior to CFY Supervision preferred Speech-Language Pathology license is required. This position is open to SLPs at all experience levels.Educational Requirements:
