JOB DUTIES
Assess home health pediatric patient for rehabilitative and functional status. Regularly re-evaluate the patients/clients speech and language need and evaluate the outcomes of care on a regular basis (If applicable). Speech Language Pathologist develops, initiates, and revises the plan of care as necessary to ensure quality and continuity of care. Identifies deficits including but not limited to communication and feeding / swallowing and potential for intervention. Evaluates the home environment. Speech Language Pathologist evaluates the need for and recommends communication devices and aides. Uses infection control measures that protect both the staff and the patient (OSHA). Coordinates services. Informs the physician and other personnel of changes in the patient's/clients condition and needs. Speech Language Pathologist demonstrates commitment and professional growth by participating in in-service programs and maintaining or improving competency. Supervises, teaches, and provides clinical direction to other speech and language personnel as required or requested. Promote Agency philosophy and administrative policies to ensure quality of care. Speech Language Pathologist provides effective communication to patients/clients, their family members, team members, and other health care professionals.
QUALIFICATIONS
Graduate of an accredited Diploma, with Bachelors, Masters, or Doctorate degree One year of experience as a Speech and Language Pathologist in a pediatric clinical care setting preferred Pediatric Home health experience preferred Reliable transportation Current Texas State license as a Speech Language Pathologist or Speech Language Pathologist Assistant Current Drivers License
