Job Summary We are seeking a passionate and dedicated Speech Language Pathologist (SLP) to join our dynamic healthcare team. In this vital role, you will assess, diagnose, and treat individuals with speech, language, voice, and communication disorders in our skilled nursing facilities. Your expertise will help improve patients' quality of life by developing personalized therapy plans and collaborating with multidisciplinary teams. Duties Conduct comprehensive speech and language evaluations for adult patients. Develop and implement individualized speech-language therapy plans tailored to each patient's unique needs and goals. Provide direct therapy services through in-person sessions ensuring accessible care for all patients. Collaborate closely with families, caregivers and healthcare professionals to support consistent progress and reinforce therapy strategies at home. Monitor patient progress regularly, adjusting treatment plans based on ongoing assessments and clinical observations. Document patient interactions, treatment outcomes, and progress notes accurately using Electronic Medical Records (EMR) or Electronic Health Records (EHR) systems. Educate patients and their families about speech and language disorders, physiology, anatomy relevant to communication issues, and strategies for improvement. Skills Strong knowledge of speech therapy techniques for individuals with speech impairments in skilled nursing facilities. Proven ability to develop and implement effective speech-language pathology plans aligned with best practices. Excellent patient assessment skills with a focus on comprehensive evaluations of speech, language, voice, fluency, cognition, and swallowing functions. Familiarity with medical terminology and physiology related to communication disorders. Experience working with individuals with disabilities or complex communication needs in multidisciplinary teams. Proficiency in using EMR/EHR systems for documentation and patient record management. Ability to collaborate effectively with families, healthcare providers, and other stakeholders to ensure holistic care. Knowledge of anatomy related to speech production and swallowing functions enhances diagnostic accuracy and treatment planning.
