Net Health and PointClickCare (PCC) Responsibilities Evaluate residents and develop individualized treatment plans for speech, language, cognitive-communication, and swallowing disorders Provide evidence-based therapy interventions designed to maximize independence and function Collaborate with physicians, nursing staff, therapy team members, residents, and families to achieve optimal outcomes Complete accurate and timely documentation in Net Health and PCC Participate in interdisciplinary meetings, care planning, and discharge planning activities Maintain compliance with all facility, state, and federal regulations Qualifications Master's Degree in Speech-Language Pathology Current Vermont Speech-Language Pathology License Certificate of Clinical Competence (CCC-SLP) required Experience in skilled nursing, rehabilitation, or long-term care preferred Strong communication, organizational, and clinical skills Passion for providing resident-centered care Why Join St. Johnsbury Center for Living & Rehabilitation?
