SUMMARY Plan, organize, develop and implement a progressive therapy program geared to improvement of resident's physical functions, psychological, social and economic well being. Follow corporate guidelines, objectives and policies to maintain standards of treatment consistent with legal and accrediting requirements.
ESSENTIAL DUTIES AND RESPONSIBILITIES
include the following. Other duties may be assigned.ASSESSMENT 1.
Upon receiving SLP orders, reviews client's history and completes thorough assessment, including precautions. 2. Upon receiving SLP screening request, reviews client's current situation and screens individuals for intervention. 3. Communicates assessment/screening results to client and appropriate family and/or staff. 4. Completes required assessment/screening documentation.PLANNING 1.
Identifies and prioritizes needs based on assessment and establishes appropriate plan with goals. 2. Communicates treatment plan and goals to client and family. 3. Collaborates with health care team members to begin planning for appropriate and timely discharge or transfer.IMPLEMENTATION 1.
Demonstrates ability to prioritize treatment procedures according to client's needs and MD requests. 2. Implements interventions according to plan of care. 3. Communicates purpose of treatment procedures to client/family according to client's level of understanding and modifies plan accordingly. 4. Communicates ongoing treatment issues/plan to interdisciplinary health care team members. 5. Completes required documentation regarding daily intervention.EVALUATION 1.
Reviews and evaluates effectiveness of interventions toward achieving client outcomes. 2. Documents appropriate reassessment and changes in plan of care. 3. Communicates significant changes in intervention to client/family and health care team members. 4. Completes required documentation for discharge.PROGRAM DEVELOPMENT 1.
Under the supervision of the Lead Therapist and/or the Director of Rehab, develops and implements new programs to enhance effectiveness of Rehab Program; reviews existing programs on an ongoing basis. 2. Develops, implements and supervises student practicum under the supervision of the Lead Therapist and/or Director of Rehab. 3. Develops and presents orientation/inservice training for facility employees and community participants. 4. Assists in developing and maintaining Rehab policies and procedures.PROFESSIONAL SELF-GROWTH 1.
Attends at least one continuing education course annually and presents information to staff within four weeks of attending course. 2. Attends at least three facility inservices annually including all mandatory inservices. 3. Participate in organizational committees.OTHER DUTIES 1.
Performs other related duties as assigned. 2. Supports and participates in changes and suggestions made by the Director of Rehab and/or Lead Therapist.SUPERVISORY RESPONSIBILITIES
