Sheridan, Wyoming Work setting: LTC•Nursing Home Responsibilities Conduct comprehensive speech, language, voice, fluency, and swallowing assessments for residents in the long-term care setting. Develop and implement individualized treatment plans to improve or maintain communication, swallowing, and related functional goals. Provide direct therapy to residents, document progress, and adjust plans based on ongoing assessment and multidisciplinary input. Maintain accurate and timely documentation of evaluations, treatment sessions, progress notes, and discharge planning. Collaborate with an interdisciplinary care team (nurses, therapists, and support staff) to ensure safety and continuity of care. Educate residents, families, and care partners about communication strategies, swallowing precautions, and home exercise programs as appropriate. Requirements Active Speech-Language Pathology license to practice in Wyoming or eligible for licensure in Wyoming. Ability to work in a long-term care environment on the listed schedule. Demonstrated communication and collaboration skills for working with a multidisciplinary team. Experience or comfort providing direct therapy and documenting progress in a skilled nursing/long-term care setting is preferred.
