This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or memory care. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts when necessary Observe patients closely for changes in condition, vital signs, and overall well-being, documenting findings accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Administer medications following prescribed instructions while maintaining strict adherence to HIPAA regulations for patient privacy and confidentiality Support clients with meal preparation, feeding, laundry, light cleaning, and maintaining a safe living environment Monitor and assist with specialized care needs such as catheter care, Alzheimer's or dementia care, hospice support, and working with individuals with developmental disabilities or disabilities Use medical terminology appropriately when documenting medical records or communicating with healthcare professionals Employ CPR certification skills during emergencies to ensure client safety and respond effectively to urgent situations Experience Prior experience in assisted living facilities, nursing homes, home health agencies, or long-term care environments is highly preferred Proven ability to work with individuals with disabilities or chronic conditions such as dementia or Alzheimer's disease Familiarity with EMR/EHR systems for accurate documentation of patient care activities Experience providing medication administration and vital signs monitoring in compliance with healthcare standards Knowledge of medical procedures including catheter care, hospice support techniques, and use of mobility aids like Hoyer lifts Demonstrated caregiving skills in home & community settings with a focus on patient safety and well-being Strong understanding of medical terminology and HIPAA regulations to ensure professional communication and confidentiality Join us to empower individuals through dedicated caregiving that enhances quality of life.
