Job Description

Job Summary We are seeking compassionate and dedicated Home Care Aides to provide exceptional in-home care to clients across various needs. This role involves supporting individuals with daily activities, ensuring their safety, and promoting independence in a respectful and caring manner. The ideal candidate will have experience in senior care, home health, or long-term care environments, with a strong focus on patient-centered service and adherence to healthcare regulations. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient conditions, reporting any changes or concerns promptly Administer medications and manage medical documentation accurately using EMR/EHR systems Support clients with meal preparation, feeding, and nutritional needs while adhering to dietary restrictions Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment Utilize medical equipment such as Hoyer lifts, catheters, and vital signs monitors safely and effectively Provide companionship and emotional support to enhance quality of life for clients with dementia or Alzheimer's disease Document patient care activities in compliance with HIPAA regulations and organizational policies Assist with hospice care or memory care as needed, respecting the dignity and preferences of each individual Skills Proven experience in home health, senior care, or long-term care environments Knowledge of medical terminology, medication administration, and medical documentation procedures Ability to work with individuals with developmental disabilities or disabilities requiring specialized care such as dementia or Alzheimer's disease Familiarity with EMR/EHR systems for accurate record keeping Strong patient observation skills to identify changes in condition or behavior CPR certification and experience with hospice or palliative care preferred Ability to operate medical equipment like Hoyer lifts and perform catheter care safely Compassionate caregiver with excellent communication skills and patience Ability to work independently while following healthcare protocols and safety standards Experience working within HIPAA guidelines to ensure patient confidentiality This position offers an opportunity to make a meaningful difference in the lives of clients while working in a supportive environment that values professionalism and compassion.
