Horizons Home Care of Holiday Health Care is adding a home health nurse to our growing team! Horizons Home Care is a locally owned and operated home health company offering in-home care to individuals throughout the tri-state area. The home health nurse will provide quality, skilled nursing in accordance with the orders of a physician and under a plan of care in a patient's home. The nurse will have the responsibility of observing, assessing, nursing diagnosing, counseling, caring, and teaching.

The Heritage Center & Horizons Rehabilitation Qualifications:

Must be a graduate of a registered nursing program from an accredited school Must possess and maintain a current, and in good standing, license with the Indiana State Board of Nursing Must have a valid drivers license, auto insurance, and the ability to drive to patient residencies
