Job Description

TITLE:
Home Care
LPN/RN REPORTS TO
Director of Nursing Job Summary:
The Home Care Nurse will provide nursing services and care coordination for home care clients to ensure safe, effective, and compliant care. Responsible for coordinating client admissions and discharges, completing OASIS assessments and updating plans of care (RN only). LPN would be assisting by collecting and reporting assessment data.
Qualifications :
Must have a current/active LPN or RN license with the State of Minnesota. Must keep up with CEU's for license renewal. The Home Care Nurse is also required to have an ability to read, write and speak English, and complete and pass a state required background study form which checks for certain criminal convictions or serious/recurring maltreatment. $25.00-$41.00 wage range
General Responsibilities:
Conduct or assist with Oasis assessments and update client Plan of Care. Assist with new hire orientation. Conduct Staff training and education annually and as required. Supervise Home Health Aides. Update Home Health Aide care plans and annually complete chart reviews. Contacting Primary Care Provider's and Pharmacy as needed. Schedule client medical appointments. Set up client medication and administer medications, including but not limited to monthly injections managing client glucose meters and completing client nail care. Review and verify narcotic control book to ensure accurate documentation and compliance. Ensure proper organization, filing, and maintenance of paper charts in compliance with state regulations, agency policies, and HIPAA requirements. Assist in investigations regarding Vulnerable Adult issues and appropriate reporting if needed. Follow and abide by the HIPAA (Health Insurance Portability and Accountability Act) privacy policy as administered by Clarkfield Care Center and Home Health Agency. Other duties as assigned.
