Home Care Nurse (LPN, RN)

Age in Place Home Care is looking for a reliable and compassionate Home Care Nurse (LPN/RN) to join our team. The nurse will perform routine visits to the patients' homes throughout Shamokin, PA and surrounding areas to ensure quality of care. This is a flexible , part-time position that is perfect for an applicant looking to create their own schedule! Responsibilities Perform an initial assessment of patient's care Perform routine 90 care visits to reassess and reevaluate the care of patient Monitor patient's condition and assess their needs to provide the best possible care and advice Report observations and any change in patient's symptoms or care to the agency Observe and interpret patient's symptoms Collaborate with care team to devise individualized care plans for patients Inspect the facilities and act to maintain excellent hygiene and safety Foster a supportive and compassionate environment to care for patients and their families Skills Proven experience as a LPN/RN Experience in home care nursing Excellent knowledge of nursing care methods and procedures In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times A team player with excellent communication and interpersonal skills BSC or other diploma from a nursing program Valid LPN/RN license Compensation Compensation is tied to admission and visitation

RN:

$25 each visit/ $45 each admission

LPN:

$25 each visit/ $35 each admission Estimated 10 hour compensation: $200-$400

Mileage Reimbursement Job Types:
