Job Description
Job Summary:
Review home care and hospiceSN, AIDE, MSW
daily notes for accuracy as needed. Analyze data to present quarterly QAPI data for home care and hospice. Provide feedback to home care and hospice supervisors for needed QA improvements. Attend education classes, IDT, and case conference as needed. Assist hospice and home care teams as needed in day-to-day operations. Provide one-on-one education/QA review with appropriate clinicians. Demonstrate and encourage team behavior and exceptional patient/guest experiences. Uphold and promote patient safety and quality.Education:
License in practical nursing or associate degree in nursing required.Experience:
Minimal 2 years' experience as home care field nurse required. Preferred experience in hospice. Preferred hospital med/surg; ER or critical care experience.Additional Skills/Abilities:
Leadership skills; great communication skills; critical thinking skills; knowledge in use of Microsoft and Excel; flexibility and ability to multitask and prioritize. Must be organized and driven to complete tasks by a deadline.HOME CARE QUALITY COORDINATOR 3.3 3.3
