This dynamic role offers the opportunity to provide comprehensive, compassionate nursing care, ensuring patients receive tailored treatments and support to promote health, independence, and well-being. As a vital part of our home health services, you will coordinate care plans, educate patients and families, and collaborate with interdisciplinary teams to deliver exceptional patient-centered care. Responsibilities Conduct thorough patient assessments including vital signs, physical examinations, and detailed health histories to develop personalized care plans. Administer medications via injections, IV infusions, and other routes while adhering to aseptic techniques and safety protocols. Manage complex medical procedures such as catheterization, wound suturing, ostomy care, and ventilator management in a home setting as needed. Monitor patient progress through regular evaluations, adjusting care plans as needed to optimize outcomes for conditions like chronic illnesses, post-acute recovery, or palliative needs. Educate patients and their families on medication administration, disease management including infectious disease care and use of medical devices like DME (durable medical equipment). Utilize electronic health record (EHR) systems for accurate documentation of patient care activities and medical records compliance with HIPAA regulations. Collaborate with physicians, therapists, case managers, and other healthcare professionals to coordinate services across settings including assisted living facilities, nursing homes, or outpatient clinics. Experience Proven clinical experience in home care nursing or related settings such as hospital medicine, or long term care. Strong background in managing acute pain, trauma medicine, behavioral health issues, geriatrics including memory care and working with individuals with disabilities or developmental challenges. Knowledge of advanced nursing procedures including IV insertion and infection control practices. Excellent communication skills for patient education in diverse populations including adults with complex needs, or elderly patients requiring hospice & palliative care.
