Job Description
Job Overview Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of individuals needing support at home. In this vital role, you will provide personalized assistance with daily activities, monitor patient health, and ensure a safe, comfortable environment. This position offers the opportunity to work closely with patients, families, and healthcare professionals in a rewarding and dynamic setting. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates accurately using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems Support medication administration following prescribed protocols while maintaining strict compliance with HIPAA regulations to protect patient confidentiality Observe and report changes in patient condition, including symptoms related to dementia, Alzheimer's disease, or other chronic illnesses Provide specialized care such as catheter care, hospice support, memory care, and working with individuals with developmental disabilities or disabilities Assist with medical procedures like Hoyer lift transfers and basic wound care as trained Prepare nutritious meals tailored to dietary needs and assist with light housekeeping tasks such as cleaning and laundry to maintain a safe living environment Document all patient interactions thoroughly in medical records and ensure compliance with healthcare standards and safety protocols Experience Prior experience in home health care, assisted living facilities, nursing homes or long-term care environments is highly preferred Familiarity with medical terminology and medical documentation practices is essential Proven ability to work effectively with individuals experiencing dementia or Alzheimer's disease, memory care needs or hospice situations Experience working with individuals with developmental disabilities or disabilities is advantageous Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as catheter care or hospice care are a plus Demonstrated competence in using EMR/EHR systems for accurate record keeping Strong understanding of patient safety protocols including vital signs monitoring and medication administration Ability to perform physical tasks such as lifting using Hoyer lifts safely and effectively while maintaining patient dignity Join us if you're passionate about providing compassionate home health services that empower individuals to live comfortably and independently.
