Job Description
Job Summary Join our compassionate team as a Home Health Aide, where your dedication and caregiving skills will make a meaningful difference in the lives of individuals requiring assistance with daily activities and health management. In this vital role, you will provide personalized home care services, ensuring safety, comfort, and dignity for clients across diverse needs. Your energetic approach and attention to detail will foster a supportive environment that promotes independence and well-being. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor and observe patients' health status, recording vital signs and reporting changes promptly. Administer medication and perform basic medical tasks following healthcare provider instructions. Support patients with specialized care needs such as Alzheimer's, dementia, memory care, hospice, or developmental disabilities. Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document care activities accurately and securely in compliance with HIPAA regulations. Use assistive devices like Hoyer lifts safely to prevent injury during transfers. Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning, and meal preparation. Collaborate with healthcare teams to ensure comprehensive patient care plans are followed effectively. Assist with medical procedures such as catheter care Provide emotional support and companionship to promote mental well-being in a home or community setting. Qualifications Proven experience in home health care, assisted living, nursing homes, or long-term care environments. Hold a current Michigan CNA certificate Strong knowledge of medical terminology and patient care procedures. Certification in CPR and first aid required; additional certifications such as hospice or dementia care are a plus. Experience working with individuals with disabilities or developmental challenges preferred. Ability to accurately monitor vital signs and recognize signs of health deterioration. Familiarity with EMR/EHR systems for medical documentation. Excellent communication skills to interact effectively with clients, families, and healthcare professionals. Valid driver's license and reliable transportation are often necessary for community-based roles.
