This vital role involves providing attentive, respectful, and comprehensive support to promote independence, safety, and well-being. As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver high-quality home health services that enhance quality of life. This position offers an engaging opportunity to develop your caregiving skills while fostering trusting relationships with those you serve. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patients' health status, including vital signs and overall condition Administer medication reminders and assist with medication administration as directed by healthcare providers Support clients with specialized needs such as Alzheimer's care, dementia care, memory care, and hospice care Operate Electronic Health Record (EHR) systems to document medical records accurately and maintain compliance with HIPAA regulations Use medical terminology appropriately to record observations and communicate effectively with healthcare teams Employ assistive devices such as Hoyer lifts and perform catheter care safely when required Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and preparation Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for clients Support clients in community outings or errands to promote social engagement and independence Requirements Proven experience in home health care, assisted living, nursing homes or long-term care environments Knowledge of medical terminology, vital signs monitoring, medication administration, and documentation procedures Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice or Alzheimer's care are a plus Ability to work with people with disabilities or developmental disabilities with patience and empathy Familiarity with EMR (Electronic Medical Records) systems or EHR (Electronic Health Records) systems for accurate record keeping Experience working with seniors or in home & community care settings preferred Strong communication skills to collaborate effectively with healthcare teams and families Physical ability to lift or transfer patients using equipment like Hoyer lifts; capable of performing patient observation duties throughout shifts Commitment to maintaining patient privacy and confidentiality in accordance with HIPAA regulations Join us in delivering compassionate home health services that truly make a difference!
