Job Overview Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and proactive support will make a meaningful difference in the lives of individuals requiring assistance with daily activities and health management. In this vital role, you will provide personalized in-home care, ensuring safety, comfort, and dignity for your clients. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Monitor and observe patients' health status by recording vital signs such as blood pressure, pulse, temperature, and respiration. Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations and medical documentation standards. Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to accurately document patient information and care activities. Provide specialized care for clients with dementia or Alzheimer's disease, including memory care strategies and behavioral management techniques. Assist with catheter care, wound dressing changes, Hoyer lift transfers, and other medical procedures as trained. Support clients in assisted living or long-term care settings, including hospice or palliative care when needed. Prepare nutritious meals tailored to dietary restrictions and assist with feeding when necessary. Perform light housekeeping duties such as cleaning, laundry, and maintaining a safe environment to promote overall well-being. Support clients' emotional needs through active listening, companionship, and engaging activities that promote mental health. Collaborate with nursing staff and other healthcare professionals to ensure comprehensive patient care plans are followed effectively. Qualifications Proven experience in home health care, nursing home settings, or assisted living environments is highly preferred. Strong knowledge of medical terminology and clinical procedures such as vital signs measurement, medication administration, catheter care, and dementia or Alzheimer's care. Demonstrated ability to work with people with developmental disabilities or disabilities requiring specialized support. Valid CPR certification and knowledge of emergency response protocols are required. Experience working with EMR/EHR systems for accurate medical documentation is essential. Familiarity with hospice care practices and long-term care environments enhances your candidacy. Excellent patient observation skills combined with compassionate caregiving attitude to ensure safety and comfort at all times. Ability to perform physically demanding tasks such as lifting using Hoyer lifts or assisting with mobility while maintaining safety standards. Strong communication skills to effectively interact with patients, families, and healthcare teams while respecting privacy laws like HIPAA. Flexibility to work live-in shifts or varied schedules based on client needs is advantageous.
