Job Description

Overview Join our compassionate team as a Home Health Aide and make a meaningful difference in the lives of individuals requiring personalized care and support. In this vital role, you will provide essential assistance to clients in their homes or community settings, promoting independence, safety, and well-being. Your dedication will help enhance quality of life for seniors, individuals with disabilities, or those recovering from illness, all while working in a supportive and dynamic environment. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient condition through observation of vital signs and overall health status Administer medications and perform basic medical tasks such as catheter care, wound care, and hospice support under supervision Utilize Electronic Health Record (EHR) systems to document patient information accurately and securely in compliance with HIPAA regulations Support clients with meal preparation, feeding, laundry, light cleaning, and household chores to maintain a safe and comfortable environment Operate medical equipment such as Hoyer lifts and assist with transfers using proper techniques to ensure safety Provide companionship and emotional support while engaging clients in activities tailored to their needs, including dementia or Alzheimer's care and memory support Collaborate with healthcare professionals by maintaining detailed medical documentation and reporting changes in patient condition promptly Work with individuals with developmental disabilities or disabilities by implementing personalized care plans in home or community settings Follow infection control protocols diligently to ensure a safe environment for clients and staff Qualifications Prior experience in home health care, assisted living, nursing homes, or long-term care environments is highly desirable Knowledge of medical terminology, medication administration, vital signs monitoring, and basic nursing skills such as catheter care and hospice support Experience working with individuals with disabilities or developmental challenges is preferred Ability to operate EMR/EHR systems efficiently for accurate recordkeeping Strong communication skills to interact effectively with clients, families, and healthcare teams Compassionate attitude with a commitment to providing respectful patient care in home and community settings Join us to be part of a dedicated team that values your skills and passion for caregiving.
