Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary. Monitor and observe patients' conditions, accurately recording vital signs and changes in health status to ensure timely interventions. Administer medications and perform basic medical procedures including catheter care, wound care, and hospice support under supervision or as trained. Maintain detailed medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring HIPAA compliance at all times. Support clients with meal preparation, light cleaning, laundry, and maintaining a safe and clean environment conducive to health and well-being. Provide specialized care for individuals with dementia, Alzheimer's disease, developmental disabilities, or those requiring memory care or hospice services. Collaborate with healthcare professionals to coordinate patient care plans while respecting patient privacy and confidentiality standards. Experience Prior experience in home health care, assisted living facilities, nursing homes or long-term care environments is highly preferred. Demonstrated ability to work with individuals with disabilities or chronic conditions such as dementia or Alzheimer's disease. Familiarity with medical terminology, vital signs monitoring, medication administration, and medical documentation practices. Experience using EMR/EHR systems for accurate record keeping and communication among healthcare teams. Certification in CPR is required; additional certifications such as hospice care training or catheter care are a plus. Proven ability to handle sensitive situations with compassion while maintaining professionalism in patient care settings. Strong communication skills combined with a proactive approach to caregiving ensure excellent patient outcomes.
