Job Description

About the RoleThe Home Health Aide plays a vital role in providing compassionate and personalized care to individuals in their own homes, ensuring their comfort, safety, and well-being. This position focuses on assisting clients with daily living activities, managing their health needs, and supporting their independence in a familiar environment. The aide will work closely with healthcare professionals and family members to implement care plans tailored to each client's unique requirements. By delivering attentive and respectful care, the Home Health Aide helps improve the quality of life for clients while promoting dignity and respect. This role requires a commitment to patient-centered care and the ability to adapt to the evolving needs of clients in the Indy South community.

Job ResponsibilitiesAssist clients with personal care tasks such as bathing, dressing, grooming, and toileting to maintain hygiene and comfort.

Support clients with mobility and transfers to prevent falls and promote physical activity within their capabilities.

Monitor and report changes in clients' health status or behavior to healthcare providers and family members promptly.

Help with medication reminders and adherence according to prescribed schedules and care plans.

Perform light housekeeping duties, meal preparation, and errands to create a safe and supportive home environment.

Provide companionship and emotional support to reduce feelings of isolation and enhance mental well-being.

Maintain accurate documentation of care provided and communicate effectively with the care team.

Minimum QualificationsHigh school diploma or equivalent.

Completion of a state-approved Home Health Aide training program.

Certification as a Home Health Aide or equivalent credential as required by state regulations.

Ability to pass background checks and health screenings.

Valid driver's license and reliable transportation to travel within the Indy South area.

Preferred QualificationsPrevious experience in assisted living, long-term care, or home care settings.

CPR and First Aid certification.

Familiarity with electronic health records and documentation systems.

Strong communication skills and ability to work collaboratively with multidisciplinary teams.

Additional training in specialized care areas such as dementia or chronic disease management.

Patient care skills ensure that aides can provide safe and effective assistance with daily living activities while respecting client dignity. Experience in assisted living and long-term care environments equips the aide with knowledge of best practices and regulatory standards.
