Home Health Aide (HHA)

Job Summary:

3 positions open in the St. Johns area Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or transfer belts. Monitor patient observations such as vital signs, mental status, and overall health to ensure safety and detect any changes requiring medical attention. Support medication administration and management in accordance with healthcare plans while maintaining accurate medical documentation. Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and behavioral support strategies. Assist with medical procedures such as catheter care, wound dressing, and hospice care as directed by healthcare professionals. Maintain detailed records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient progress and care provided. Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment aligned with HIPAA privacy standards. Support clients in community settings by helping them participate in social activities and ensuring their comfort during home & community care engagements. Experience Previous experience working as a Home Health Aide (HHA), Nursing Assistant, or in long-term care environments such as nursing homes or assisted living facilities. Proven ability to work effectively with individuals who have developmental disabilities or cognitive impairments like dementia or Alzheimer's disease.
