Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs. The Alton Intermittent Home Care Department of BJC Home Care Services provides home visits to patients in Alton, Illinois and several nearby counties, with 24 hour on-call home care nursing supervision. Preferred Qualifications Role Purpose The Certified Nurse Assistant is responsible for providing direct patient/resident care and assistance in accordance with all applicable policies, procedures, regulations and nursing standards of care. Responsibilities Obtains vital signs and other measurements as directed and documents findings promptly. Meets patient hygiene, comfort, nutrition, and hydration needs and documents as required. Recognizes and reports abnormal findings or changes in patient status to the nurse throughout entire shift. Implements care under the supervision of the nurse.or Primary Case Manager. In Home Health, this can be done with a RN/PT/OT or Speech Pathologist.
