Home Health & Hospice Social Worker

Description :

The Home Health & Hospice Social Worker is responsible for providing hospice social work services and counseling to patients and family units served by Orange City Area Home Health & Hospice. The purpose of this position is to provide initial psychosocial evaluations, ongoing psychosocial counseling, direct case work services, bereavement services, and community education, outreach and referral. He/she also coordinates the Hospice volunteers and the day to day provision of volunteer care. The hospice social worker is an integral part of the Hospice interdisciplinary team (IDT).

Number of Hours :

Part- Time 24 hours/week

Schedule :

Tuesday, Wednesday, & Friday, although there is some flexibility in the schedule and days may vary depending on need.

Educational Requirements :

Bachelor's degree in social work from an institution accredited by the Council on Social Work Education; or a baccalaureate degree in psychology, sociology, or other field related to social work and one year of experience in healthcare.

Other:

He/she must be able to see patients and families within a 30 mile radius of Orange City.
