Job Description
We are seeking a detail-oriented and organized Home Health Intake Coordinator to join our team. This role is essential in ensuring a smooth and efficient intake process for patients entering home health services. While your designated home office will be based in Charlevoix, this position will require you to work on-site in our Lake City office 8a-5p.
Job Details:
Manage and coordinate all aspects of patient intake for home health services Receive and process referrals from hospitals, physicians, and other healthcare providers Verify patient insurance and eligibility Maintain accurate and timely documentation in electronic health record systems Communicate effectively with clinical staff, referral sources, and patients/families as well as schedule the visits. Ensure compliance with all regulatory and agency requirements Provide excellent customer service and act as a liaison between departments Requirements High School Diploma or GED Ability to handle confidential information with professionalism Experience is preferred.
