Home Health LPN

The Home Health LPN is an integral part of our inter-disciplinary care team that provides care to patients in their home. We pay hourly, offer a 32 - 40 hour work week with flexible AM start time to support work/life balance. We are currently hiring full-time LPNs in the Milford area

Licensed Practical Nurse Responsibilities:

Making home visits to patients living in designated territories. Perform assigned duties, including medication administration, wound care, IV therapy, treatments, and procedures. Teach and educate patients and/or caregivers on disease process, medications, diet, wound care and other treatments as ordered by the physician. Communicates regularly with the RN Case Manager and the patient inter-disciplinary care team to ensure the most appropriate care is being provided. Licensed Practical Nurse:

A current license as a Licensed Practice Nurse in the state of Massachusetts.
