Key Responsibilities:
Patient Care:
- Provide direct nursing care to patients, including administering medications, wound care, and other treatment modalities as prescribed by the physician. Monitor and record vital signs, including temperature, blood pressure, pulse, and respiration rates, and report any irregularities to the healthcare team.
Care Planning:
- Collaborate with Registered Nurses (RNs) and other healthcare professionals to develop and implement individualized care plans. Participate in patient assessments to determine ongoing care needs and contribute to the development of patient care plans.
Patient Education:
- Educate patients and their families on disease management, treatment plans, and home healthcare procedures. Provide guidance and support to promote patient independence and adherence to prescribed therapies.
Documentation:
- Maintain accurate and timely documentation of patient care activities, observations, and progress notes in accordance with regulatory requirements and company policies. Ensure all records are complete and accurate to support continuity of care.
Collaboration:
- Work closely with a multidisciplinary team, including RNs, therapists, social workers, and other healthcare professionals, to ensure comprehensive and coordinated care delivery. Participate in team meetings and contribute to case discussions and care planning.
Compliance:
- Adhere to all applicable healthcare regulations and standards, including infection control, patient safety, and privacy practices. Ensure compliance with state and federal laws, as well as company policies and procedures.
Emergency Response:
- Respond promptly to patient emergencies and follow established protocols to ensure patient safety. Notify appropriate personnel and provide accurate and timely information during emergency situations.
Qualifications:
- Current and valid LPN or LVN license in the state of employment.
- Previous experience in home healthcare or a similar clinical setting is preferred.
- Strong clinical skills and the ability to perform a variety of nursing procedures.
- Excellent communication and interpersonal skills to effectively interact with patients, families, and the healthcare team.
- Ability to work independently, manage time effectively, and prioritize tasks in a home healthcare environment.
- Compassionate, patient-centered approach with a focus on delivering quality care.
- Basic computer skills for documentation and electronic health records management.
Physical Requirements:
- Ability to perform tasks that may include lifting and transferring patients.
- Capacity to handle various physical demands, such as standing, walking, bending, and reaching for extended periods.
- Good visual and auditory acuity for observing and assessing patients' conditions.
