Valid driver's license and proof of car insurance Administers and writes evaluations of assigned patients in accordance with agency policy and procedures Must be a licensed Occupational Therapist by the state Maintains timely and accurate patient records and reports Must have at least one-year clinical experience Checks in on Certified Occupational Therapy Assistants after every 4-6 patient visits Must be able to travel to prospective patient's place of residence

Compensation:

$60-$115 per visit
