1 Job Description McLeod Home Health Care Services is seeking an experienced Home Health Physical Therapist (PT) to provide comprehensive in-home physical therapy services to patients in accordance with physician orders and individualized treatment plans. The Physical Therapist will work closely with patients, families, physicians, and interdisciplinary healthcare teams to promote patient recovery, improve mobility, restore function, and enhance quality of life in the home health setting. The ideal candidate will demonstrate strong clinical judgment, independence, excellent communication skills, and the ability to manage a patient caseload efficiently while maintaining compliance with home health regulations and agency standards. Responsibilities Provide skilled physical therapy services in patients' homes according to physician orders and individualized care plans Perform comprehensive patient evaluations and develop appropriate treatment plans Assess patient mobility, strength, balance, endurance, and functional limitations Implement therapeutic exercises, mobility training, gait training, and pain management interventions Educate patients and caregivers regarding treatment plans, home exercise programs, and safety techniques Monitor patient progress and adjust treatment plans as necessary Maintain accurate and timely clinical documentation in compliance with agency policies and regulatory requirements Coordinate care with physicians, nurses, case managers, and interdisciplinary healthcare teams Demonstrate knowledge of home health regulations, Conditions of Participation, and state therapy laws Participate in professional development activities, competency training, and performance improvement initiatives Function independently while effectively managing time, scheduling, and patient care responsibilities Ensure patient safety and maintain infection prevention standards during all visits Requirements Active Physical Therapist (PT) License required Previous Home Health Physical Therapy experience preferred Strong understanding of home health regulations and patient care standards Ability to work independently and manage a flexible patient schedule Excellent communication, customer service, and interpersonal skills Ability to perform physical tasks including standing, walking, bending, lifting, and driving regularly throughout the workday Strong clinical assessment and documentation skills Valid driver's license and reliable transportation required Why Join This Assignment?
