Home Health Physical Therapist

We are looking for a Physical Therapist to join our team and provide home health services to our patients. The successful candidate will be responsible for assessing patient needs, developing and implementing treatment plans, and monitoring progress. The ideal candidate should have excellent communication skills, be able to work independently, and have a passion for helping others. Responsibilities:

Evaluate patient's physical condition and develop a plan of care. Provide home health physical therapy services to patients in their homes. Instruct patients and family members in proper exercise techniques and safety precautions. Assess patient's progress and modify treatment plans as needed. Document patient's progress and treatment plans in accordance with professional standards. Provide education to patients and family members on the importance of physical therapy. Maintain professional relationships with other health care providers.

$1,954.00 - $2,032.00 per week
