Job Summary Join our dynamic healthcare team as a Home Health Registered Nurse on a 13-week contract, where you'll deliver compassionate, expert nursing care directly in patients' homes. PM NO CALL Service Area:
Rhinelander Must have at least 1 year of Home Health experience within the last 3 years HCHB and Oasis is required Responsibilities Conduct thorough patient assessments to evaluate health status, needs, and home environment suitability. Develop and implement individualized care plans aligned with physician orders and patient goals. Administer medications, including injections and IV infusions, following aseptic techniques and safety protocols. Monitor vital signs, manage wounds, perform suturing when necessary, and provide advanced clinical interventions such as ventilator management or ostomy care. Educate patients and their families on disease management, medication administration, nutrition, and self-care techniques to promote health literacy. Collaborate with interdisciplinary teams utilizing EMR (Electronic Medical Records) systems like Epic or Cerner to document care accurately and efficiently. Coordinate discharge planning and follow-up care to ensure seamless transitions between settings while adhering to HIPAA regulations for patient privacy. Requirements Valid RN license in the state of practice with current CPR/BLS certification. Proven experience in home health nursing or related settings such as hospital medicine, ICU, or emergency room environments. Strong knowledge of medical terminology, physiology, and diagnostic evaluation methods including ICD coding (ICD-10/ICD-9). Hands-on experience with IV insertion, medication administration, catheter care, phlebotomy, and aseptic technique. Familiarity with EMR/EHR systems like Epic or Cerner for accurate documentation and case management. Critical care experience preferred—especially in ICU or Level I/II trauma centers—and experience working with individuals with disabilities or complex chronic conditions such as dementia or memory care needs. Ability to perform physical examinations, vital sign assessments, specimen collection/processing, and infection control procedures confidently. Excellent communication skills for triage calls, patient education sessions, and interdisciplinary collaboration. Knowledge of Workers' Compensation law related to occupational health cases is a plus; experience in hospice & palliative medicine or behavioral health is beneficial.
