Home Health Registered Nurse

• This is for the Full-time position This position will cover Lake County , The Villages , and the surrounding area.
• Provide direct patient care in a home health setting
• Perform comprehensive assessments of the patient's health status
• Develop and implement individualized care plans
• Administer medications and treatments as prescribed by physicians
• Monitor and evaluate the patient's progress and make necessary adjustments to the care plan
• Educate patients and their families on managing their health conditions
• Coordinate with other healthcare professionals to ensure continuity of care
• Document all patient interactions and interventions accurately and timely
• Attend case conferences to provide and get input from other healthcare professionals to ensure the best possible care for patients

Qualifications and Requirements:

• Valid Florida Registered Nurse (RN) license
• Must have a valid FL Driver's License
• Must have current CPR Card
• Must have current AHCA Background Screening Report
• Must have either Chest XR report , 2 Step TST report, orTB Lab Test Report
• ( Quantiferon Gold )
• Must have a Doctor's Certificate of Good Health
• Strong knowledge of home health regulations and best practices
• Excellent clinical assessment and critical thinking skills
• Ability to work independently and manage time effectively
• Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members
• Proficient in using electronic medical record (EMR) systems
