Equity Staffing is seeking a skilled and compassionate Registered Nurse (RN) for a home health opportunity in the Summerville, WA area. This role is ideal for an experienced RN who is comfortable managing admissions, ongoing patient assessments, and recertifications in a fast-paced field environment.

Position Details:

Weekly Hour Guarantee:

36 hours

Schedule:

Days, evenings, weekends with rotating call coverage

On-Call Requirement:

Approximately every 3-4 weeks when fully staffed

Key Responsibilities:

Complete Start of Care (SOC) assessments Perform Resumption of Care (ROC) visits Conduct patient recertifications and follow-up evaluations Develop and implement individualized care plans Provide skilled nursing care in the home setting Maintain accurate and timely documentation Coordinate patient care with physicians and interdisciplinary healthcare teams Monitor patient progress and adjust care plans as needed

Required Qualifications:

Active Registered Nurse (RN) license Strong clinical assessment and patient care skills Ability to work independently and manage a field-based schedule
