Job Description
Position Summary The Home Health Nurse provides skilled nursing care to patients in their homes under the direction of a physician. The nurse is responsible for assessing patient needs, developing and implementing care plans, providing education to patients and families, and coordinating services with the interdisciplinary healthcare team to promote optimal health outcomes. Essential Duties and Responsibilities
- Perform comprehensive patient assessments and document findings accurately.
- Develop, implement, and update individualized patient care plans.
- Administer medications and treatments as prescribed by the physician.
- Monitor patient progress and report changes in condition to the physician and care team.
- Provide wound care, IV therapy, catheter care, and other skilled nursing services as ordered.
- Educate patients and caregivers regarding disease management, medications, treatments, and home safety.
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
- Maintain accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
- Participate in case conferences and interdisciplinary care planning.
- Ensure compliance with all federal, state, and agency regulations related to home health services.
- Promote patient independence and quality of life through compassionate, patient-centered care. Qualifications
- Current and active Registered Nurse (RN) license in the state of employment.
- clinical nursing experience and/or home health experience preferred).
- Strong clinical assessment and critical thinking skills.
- Ability to work independently and manage time effectively.
- Excellent communication and patient education skills.
- Valid driver's license and reliable transportation. Physical Requirements
- Ability to travel to patient homes.
- Ability to lift up to 25-50 pounds as needed.
- Ability to stand, walk, bend, and perform nursing procedures. Work Environment Patient homes in a variety of community settings. Exposure to typical home health environmental conditions. Skills and Competencies
