Our team supports patients and their families with compassion, dignity, and strong clinical care during one of the most important times in their lives. We are seeking an experienced RN Admissions Nurse who is confident, compassionate, and comfortable working in the field. Role Overview As an RN Admissions Nurse, you will be the first clinical point of contact for patients and families entering hospice services. You will conduct assessments, guide the admission process, and ensure a smooth transition into care. Key Responsibilities Conduct in-home hospice admission assessments and develop initial plans of care Collaborate with physicians to confirm eligibility and coordinate admissions Educate patients and families on hospice services and care expectations Complete accurate and timely EMR documentation within 24 hours Travel throughout Orange County and Inland Empire Work closely with the interdisciplinary team to ensure continuity of care Qualifications Graduate of an accredited nursing program Active California RN license in good standing Minimum 1 year of hospice experience required Strong assessment and clinical decision-making skills Excellent communication and patient education skills Valid driver's license, reliable transportation, and auto insurance Bilingual Spanish and English is a plus Why Join Cabrillo Hospice Competitive hourly pay $62 to $67 Meaningful, patient-centered work Supportive and collaborative clinical team Autonomy in the field with leadership support Opportunity to make a real impact in patients' lives Apply If you are a compassionate RN with hospice experience and a passion for providing high-quality end-of-life care, we encourage you to apply.
