Job Description
Our office in Cassville is looking for a compassionate, organized, and dedicated Bereavement Coordinator to work with our Interdisciplinary Team in coordinating bereavement services to the client family according the individual needs. The right individual for this position will be responsible for the coordination of bereavement services in conjunction with the plan of care developed in consultation with the RN Coordinator. The bereavement services will be developed after the completion of the bereavement assessment by the Interdisciplinary Team and will be carried out for 13 months following death of the client. Key Responsibilities Develops and maintains bereavement services and makes referrals as necessary within 2 months of client death. In addition to the assessment, must make at least one bereavement visit, other than funeral attendance/visitation within 6 months after death of client. Assist in providing orientation, in-service programs and bereavement training to volunteers. Supervises volunteers and staff in bereavement services. Maintain a Bereavement follow-up log of services for 13 months following expirations of the client. Develop and implement support groups within the hospice. Participate in training events and speaking engagements for community and professional groups. Complies with accepted standards of care in accordance with Medicare guidelines and other regulatory sources.
Specialized and Professional Knowledge:
