Hospice Case Manager Shift - Monday - Friday Overview Join our compassionate team as a Hospice Case Manager, where you will play a vital role in providing comprehensive end-of-life care that emphasizes dignity, comfort, and emotional support. This dynamic position offers the opportunity to coordinate patient care plans, collaborate with multidisciplinary teams, and ensure seamless transitions across care settings. Responsibilities Conduct thorough patient assessments to determine eligibility and develop personalized care plans aligned with hospice standards. Coordinate interdisciplinary team activities, including physicians, nurses, social workers, chaplains, and therapists to deliver holistic patient care. Manage discharge planning and post-acute care arrangements to ensure continuity of services. Monitor patient progress through regular evaluations of vital signs, symptom management, and physiological responses to treatment. Educate patients and families on disease progression, medication management, and available support resources to foster informed decision-making. Maintain accurate medical records in compliance with HIPAA regulations and ensure timely documentation of patient interactions and care updates. Facilitate intake procedures for new patients, including medical record review, diagnostic evaluation, and coordination of necessary services. Requirements At least 1 year of experience with hospice case management required Experience providing hospice care in a home setting required.
