Both Pay:
$2150
  • 2250/Wk plus .42 a mile for patient related travel Local and Travel Welcome! Bedford, NH 13 Week Contracts Schedule 1: Monday
  • Friday 8-5 with backup on call as needed.
Guaranteed 40 hours per week
Service Area:
Manchester and Salem Must have at least 1 year of Hospice experience within the last 3 years. HCHB (HomeCare HomeBase)charting experience is also required. Schedule 2: Monday

Wolfeboro and Ossipee Must have at least 1 year of Hospice experience within the last 3 years. HCHB (HomeCare HomeBase)charting experience is also required. Duties Conduct thorough patient assessments to determine eligibility and develop individualized hospice care plans that align with patient needs and preferences Coordinate interdisciplinary team efforts to deliver holistic care, including symptom management, medication administration, and emotional support Monitor patient vital signs, physiological responses, and overall condition regularly to adjust care plans proactively Facilitate effective discharge planning and transition of care from hospital or inpatient settings to home or outpatient hospice services Manage medical records meticulously, ensuring compliance with HIPAA regulations and accurate documentation of patient progress and interventions Collaborate with physicians, social workers, therapists, and family members to optimize patient comfort and quality of life Perform diagnostic evaluations and coordinate post-acute care services to support recovery or end-of-life goals in various settings such as inpatient units or outpatient clinics Requirements Valid Registered Nurse (RN) license in the state of practice with current certification Critical care experience preferred, including ICU or emergency medicine backgrounds that demonstrate strong clinical judgment under pressure Experience working in Level I or Level II trauma centers, hospital medicine units, or similar acute care environments Knowledge of infectious disease management, physiology, medical terminology, and complex patient assessment techniques Prior experience in hospice, palliative care, post-acute care, or outpatient clinic settings is highly desirable Familiarity with HIPAA regulations, medical records management, discharge planning processes, and utilization management practices Strong communication skills with the ability to handle sensitive conversations compassionately while maintaining professionalism Ability to work independently within a multidisciplinary team and adapt quickly to changing patient needs Join us in delivering exceptional hospice care that emphasizes dignity, comfort, and compassion.
