Hospice Chaplain - PRN

Chaplain/Bereavement Coordinator Uplift Hospice is looking for a passionate and driven Chaplain & Bereavement Coordinator to be a core member of our amazing team! Responsibilities for this role include:

Conducting initial and follow-up Chaplain visits both in-person and via phone Conducting initial and follow-up bereavement assessments both in-person and via phone Assisting in all aspects of bereavement program from initial admission through 13-months post-passing Assisting in coordinating and hosting annual Celebration of Life ceremony Fulfilling ad hoc patient care concerns by coordinating across both internal and external stakeholders Voicing your opinion is key! PRN Pay:

$40.00 - $60.00 per hour

Education:

High school or equivalent (Preferred)
