Hospice Social Worker- PRN

Psychosocial Assessments :

Evaluate patients' and families' emotional, social, and spiritual needs to develop individualized care plans.

Emotional Support & Counseling :

Provide counseling to help patients and families cope with terminal diagnoses, grief, and bereavement.

Resource Coordination :

Assist in accessing community resources, including financial aid, home care services, and support groups.

Advance Care Planning :

Guide discussions on advance directives, DNR orders, and end-of-life preferences to ensure patients' wishes are honored.

Interdisciplinary Collaboration :

Work with healthcare teams to coordinate care, ensuring alignment with patients' goals and values.

Documentation :

Maintain accurate records of assessments, interventions, and care plans in compliance with regulatory standards.
