Job Description

Job Summary We are seeking a compassionate and knowledgeable Hospice Continuous Care LVN to join our dedicated team. The ideal candidate will possess a strong background in patient care, particularly in hospice and palliative settings. This role involves providing guidance and support to patients and their families, ensuring they receive the highest quality of care during difficult times. The Hospice Care LVN will collaborate with healthcare professionals to develop individualized care plans that address the unique needs of each patient. Duties The Continuous Care Licensed Vocational Nurse (LVN) provides direct, continuous bedside nursing care to hospice patients experiencing acute symptom crises that cannot be managed through routine care. This role supports the interdisciplinary hospice team by delivering high-quality patient-centered care, maintaining comfort, and ensuring safety in the home or facility setting. The LVN also has the option to serve as a Hospice Runner , assisting with urgent visits and interim nursing support between patient sites. Essential Duties and Responsibilities Provide continuous home care during periods of crisis (e.g., pain, respiratory distress, agitation, terminal restlessness). Deliver nursing interventions as ordered by the RN Case Manager and physician, in accordance with the hospice plan of care. Observe and document changes in patient condition and promptly communicate updates to the RN Case Manager or Clinical Manager. Administer medications safely and accurately via oral, subcutaneous, or IV routes per hospice protocol. Provide emotional support to patients and families during crisis care, ensuring dignity and comfort. Perform hospice runner duties (if applicable) , including: Responding to urgent PRN visits for symptom management. Assisting other hospice staff as needed for coverage or emergencies. Maintain accurate and timely documentation in the EMR per agency policy. Ensure infection control and safety protocols are followed at all times. Represent the hospice professionally and compassionately in the community. Qualifications Current California LVN License in good standing. Current CPR certification (BLS). Valid CA driver's license and reliable transportation. Minimum one year of hospice experience preferred. Knowledge of symptom management, comfort care, and hospice philosophy. Strong communication and critical-thinking skills. Ability to work independently with minimal supervision. Willingness to travel throughout South Orange County.
Work Schedule Flexible scheduling:
4- 8 hour shifts available. Day, night, and weekend shifts as assigned to meet patient needs. On-call or per diem availability for crisis coverage. Compensation and Benefits Competitive hourly rate (based on experience and shift). Mileage reimbursement for field travel. 24/7 clinical support and mentoring from RN leadership.
