HOSPICE Licensed Vocational Nurse

Job Summary We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including long-term care, assisted living, and acute care environments. The LVN will work under the supervision of registered nurses and physicians, ensuring that patients receive appropriate medical attention and support. Duties Administer medications and treatments as prescribed by physicians. Perform patient assessments and monitor vital signs. Assist in the development and implementation of patient care plans. Provide personal care services such as bathing, grooming, and feeding. Maintain accurate medical documentation in compliance with HIPAA regulations. Collaborate with healthcare team members to ensure optimal patient outcomes. Educate patients and their families about health management and wellness strategies. Participate in discharge planning and case management activities. Assist with IV infusion, catheterization, and other medical procedures as needed. Engage with patients experiencing developmental disabilities, Alzheimer's disease, or dementia. Experience Candidates should have experience or knowledge in the following areas: Epic EMR systems and Cerner Critical care experience, including ICU or PICU Long-term care and assisted living environments Hospital settings such as emergency medicine or internal medicine Triage and acute pain management Geriatrics, senior care, and hospice care Medical office experience with familiarity in ICD coding (ICD-9 & ICD-10) Patient assessment techniques including physical examinations and vital signs monitoring Experience working with children or in pediatrics is a plus Familiarity with medical terminology, anatomy knowledge, and physiology knowledge Kinser EMR The successful candidate will demonstrate strong communication skills, the ability to work collaboratively within a team, and a passion for providing exceptional patient care.
