Full-time Position Title:
Hospice Licensed Practical Nurse (LPN)
FLSA Status:
Non-Exempt Job Summary:
The Hospice Licensed Practical Nurse (LPN) provides direct patient care to hospice patients under the supervision of a Registered Nurse (RN) or Physician. This role involves delivering compassionate end-of-life care, assisting with care plans, and supporting patients and their families during their final stages of life.
Duties and Responsibilities:
Direct Patient Care (40%): Administer medications, treatments, and procedures as prescribed by the physician and under the direction of the RN. Monitor and record patient vital signs and report any changes in patient conditions to the RN or physician. Assist patients with activities of daily living, including bathing, dressing, and grooming. Care Plan Implementation (20%): Follow individualized care plans developed by the interdisciplinary team to meet the physical, emotional, and spiritual needs of hospice patients. Provide input to the RN and interdisciplinary team regarding patient care and progress. Ensure that all care provided is documented accurately and promptly in the patient's medical record. Patient and Family Support (15%): Provide emotional support and education to patients and their families regarding the hospice process and what to expect. Communicate effectively with patients and families to address concerns and ensure they feel supported. Assist families with the preparation for end-of-life care and decision-making. Interdisciplinary Team Collaboration (10%): Participate in interdisciplinary team meetings to discuss patient care plans and progress. Collaborate with other team members, including nurses, physicians, social workers, and chaplains, to provide holistic care. Coordinate with other healthcare providers to ensure continuity of care. Compliance and Training (10%) Follow infection control guidelines and maintain privacy rules and regulations as per HIPAA. Attend all meetings and complete in-services and mandatory training. Provide exceptional customer service and high-quality care by showing compassion and empathy towards patients and their families. Performs other related duties as assigned (5%):
Qualifications:
Education:
Completion of an accredited Licensed Practical Nurse (LPN) program.
Licensure:
Current LPN licensure in the state of practice. Must possess and maintain current CPR certification.
Experience:
Previous experience in hospice or palliative care preferred.
Skills:
Strong clinical and patient care skills. Excellent interpersonal and communication abilities. Proficiency in electronic medical records (EMR) systems and office software. Ability to work independently and as part of an interdisciplinary team. Compassionate, empathetic, and dedicated to providing high-quality patient care.
Physical Requirements:
Ability to lift up to 75 pounds and turn, move, and transport patients using proper body mechanics and lifting devices as needed. Capacity to stand, walk, and sit for extended periods. Ability to perform repetitive tasks and physical activities, such as bending, kneeling, and reaching. Work is performed in a healthcare setting with potential exposure to infectious diseases, bodily fluids, and chemicals. Must be able to work various shifts, including weekends, holidays, and overtime as needed. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Must be able to travel throughout the service area.
