Schedule:

A chance to be part of a growing hospice shaping its future . What You'll Do Travel to patient homes, assisted living, and care facilities to complete hospice admissions. Perform clinical eligibility assessments and initiate services . Lead goals-of-care conversations with patients and families. Provide clear education on hospice philosophy, services, and expectations . Coordinate with intake, physicians, and the interdisciplinary team. Ensure a smooth, confident transition into hospice care . Maintain accurate and timely documentation . What You Bring Active Minnesota Registered Nurse (RN) license. Experience in hospice, oncology, palliative care, or high-acuity hospital settings . Strong ability to communicate clearly and build trust quickly . Confident clinical judgment and decision-making skills. Comfort working independently in a field-based role . Reliable transportation and willingness to travel.
